Serving the needs of adults with special needs and their families in the Hudson County area
Parents and caregivers are at a loss once their special needs child reaches the age of 21. Parents often refer to this time as "falling off a cliff” - it occurs when individuals reach the age of 21, disqualifying them from IDEA (federal) services. As a result, they must rely on State vocational rehabilitation and Medicaid waivers to access needed resources and funding. This process is difficult, opaque and the navigation is arduous
Our team works to provide inclusive aid to children with special needs during this transition. We address the challenges through assisting with access to services (often unknown), mentoring, direct engagement with the community, legislative advocacy, consolidating resources for parents and caregivers seeking information, and launching a marketing campaign to raise awareness of this often-overlooked issue.

HSH was founded by David Tornabene, a longtime resident of Hoboken and Jersey City, and a certified nonprofit leader with years of experience guiding community nonprofit ventures.
The Home Sweet Hudson project was inspired by David's encounters with numerous friends and neighbors who were struggling to deal with special needs children who have aged out of the services they have grown up with.
His hope is to gather together a community of those interested in and affected by these issues in order to address them through direct engagement with the community, legislative advocacy on a local and state level, consolidated resources for parents and caregivers seeking information, and launching a marketing campaign to raise awareness of this often-overlooked issue.
